Team Contract, Release and Waiver of Liability, assumption of risk and indemnity and photo consent
In consideration of participating in the "Truck Pull" on July 4th 2005 in Campbellsport, I attest to the fact that I understand the nature of the event and that I am qualified, in good health, 18 years old or older and in proper physical condition to participate in such event. I promise that, in the event conditions appear unsafe, I will immediately discontinue participation in the event.
I fully understand the event involves risk of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions or inactions, those of others participating in the event, the condition in which the event takes place, or the negligence of the "releases" named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and responsibility for losses, costs and damages I incur as a result of my participation in this event.
I hereby release, discharge and covenant not to sue the Campbellsport Fireman, Campbellsport Area Chamber or Commerce, Campbellsport Building Supply Center Inc., Tom's Quality Millwork, Valley Hydro-Excavation, Koffman Auto Body, Joel Fleischman, The Village of Campbellsport, their respective administrators, directors, agents, officers, volunteers, employees, other participants, sponsors or advertisers and, if applicable, owners and lessors of premises on which the "Truck Pull" takes place (each considered one of the "RELEASEES" herein) from all liability, claims, demands, losses or damages on my account caused or alleged to be caused in whole or in part by the negligence of the "Releasees" or otherwise, including negligent rescue operations. I further agree that if despite this release, waiver or liability and assumption of risk I, or anyone on my behalf, makes a claim against any of the Releasees: I will indemnify, save, and hold harmless each of the Releasees from any loss, liability, damage or cost which may incur as a result of such claim. I have read this release and waiver of liability, assumption of risk and parental consent agreement and fully understand it. I further attest by my signature that I agree with the conditions above.
By my signature below, I also give the Campbellsport Area Chamber of Commerce and Joel Fleischman my consent to photograph and/or video me, with or without name, for educational, informational, or promotional purposes. Due to editing, I understand that I may or may not be featured in any publicity.

Rules for TRUCK PULL III
Teams are made up of 6 members (5 on the rope and 1 alternate / cheerleader). Entry fee is $100 per team. All participants will receive a truck pull t-shirt. 1st place will receive $1000 ($750 cash and $250 donated to their favorite non-profit organization), 2nd place $250, 3rd place $100. Limit of 15 teams. Non-profit organization must be have a tax exempt number to qualify.
Format based on 15 teams. (If less than 15 teams format may change and all team captains will be informed before the start of TRUCK PULL III with the revised format.) After the parade on July 4th (approx. 1:30pm) at CFD park, teams must register to begin competiton. Competition will begin after all teams are registered Each team will pull approximately 40,000lbs (the Campbellsport Building Supply Boom truck loaded with boards from Tom's Quality Millwork) a distance of up to 200'. A final trial run will be done prior to the actual pull. If a representative from your team would like to attend the practice pull contact Joel at 920-533-4412. The top 10 teams (determined by furthest length, with quickest time being the tie breaker) will advance to the 2nd round. If two or more teams pull the same length and the same time both teams will advance if they qualify. Teams will pull randomly in the first round, and by order of finish (poorest team going first) in second round and so on. Top three teams in the second round will move on to the third round. Women's competition (each women's team having one pull) will take place between the 2nd and 3rd round. Finals will determine the 1st, 2nd, and 3rd place team.
A representative from your team is encouraged to sit shotgun with the driver of the boom truck to guarantee fair play. Any rulings will be the sole discrection of the TRUCK PULL III competition committee. All decisions are final. Any team displaying poor sportsmanship may be disqualified.
$100 and a SPIRIT STICK AWARD will go to the team with the most spirit as determined by the TRUCK PULL III staff.
Truck Pull III is part of the Campbellsport Fire Dept.'s picnic and parade. NO carry-ins will be allowed in the park. Food, beverages and other entertainment are available at the park. Profits from TRUCK PULL III will go to upgrade the bathrooms and playground equipment at fireman's park. There will be a fireworks display after TRUCK PULL III at dusk.
Rain or shine teams will compete.
